District of Columbia is included separately.

STORY BEHIND THE DATA:
A 20% sample of all Medicare beneficiaries who dies between ages 66 and 99 during 2003-2007.  Excluded were beneficiaries without continuous Part A or Part B Medicare or those enrolled in Medicare HMOs in the last six months of life.  Decedents were categorized into one of 26 cancer types.
